The Newcastle Primary Care Navigation Service launched on the 31st July 2017, Simona Diciolla is the Primary Care Navigator at Throckley, Simona has a background in mental health work and will be at the practice on a Thursday. She aims to guide people 18 years and over who require guidance and support with any of the following:
- Financial (eg money/debt advice)
- Housing (eg housing advice, at risk of homelessness or are homeless)
- Lifestyle (eg diet and exercise)
- Social (eg isolation and loneliness)
- Mobility (eg falls and frailty)
- General advice/information (specify)
Patients can be referred by any member of the team, please contact the practice for more information.
