If you change name, address, telephone number (home or mobile) or e-mail address please let our receptionists know by calling or writing to us. Alternatively you can inform us of your new address/phone number/e-mail by completing the form below and your change of name by clicking here and completing an on-line form. If you move outside the practice area you will need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
