How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

Throckley Primary Care serves the areas of Throckley, Newburn, Lemington, West Denton, Blucher, Walbottle, Abbey Grange, Chapel House, Chapel Park and Heddon-on-the-Wall.  If you are unsure if your address is in our practice area please check with a Receptionist.

How you can register

You can register by bringing along your NHS medical card or completing a form available from reception.

When you apply to register as a patient with this practice, included in the questions we ask all new patients is:

• Are you a visitor?
• Is there a limit to your stay in the UK?

Providing ID and proof of address

We also request all new patients provide suitable evidence to support your application. A combination of two of the following will usually be requested as suitable forms of identification, which includes photographic ID and proof of address:

• Passport    • Driving licence
• Paid utility bills    • Local authority rent card
• Marriage certificate    • Papers from the Home Office
• Birth certificate    • Bank/building society cards/statements
• Payslip / P45 / National Insurance number card
• Letter from Benefits Agency/benefit book/signing on card

This list is not exhaustive.  Please ask at Reception if you have any queries regarding this.

Once you have registered

You will be registering with the practice rather than an individual GP.  You are free to choose to see any particular GP – although, we do suggest that (when possible) you continue to consult the same GP for any particular problem.

We ask all new patients to make an appointment with the Healthcare Assistant for a health check and with the doctor if you are on regular medications.

We prefer that children registering to join the practice have at least one parent/guardian registered/registering at the practice.
