Changes to ‘birthday checks’

If you have a long term condition (such as diabetes, COPD, asthma, high blood pressure, atrial fibrillation or have previously had a stroke or heart attack) you will be used to us asking you to come in for a yearly check around your birthday. The review is to help prevent any complications of your condition and to ensure that any complications that do occur are detected quickly and minimised.  We also explain what steps you can take to maximise your health and make sure that you are taking the correct medication.

From October your will find that the appointments will be changed slightly. You will receive a letter that asks you to make your initial appointment either with Caroline, Mandy, Pamela or Rose. You will then have a second appointment with your usual GP.

We have made this change to try to make the appointments easier for you and so that your health and medication is fully reviewed by a doctor at least once a year.
